Aluminium Hydroxide(Added as Aluminium Hydroxide Paste eqv.to
0.380 g of dried Aluminium Hydroxide gel) IP 0.291 g
Magnesium Hydroxide(Added as Magnesium
Hydroxide Paste) IP 98 mg

Description

Oxetacaine is a potent local anesthetic. It is an amide anesthetic that is stated to have prolonged action. It is administered by mouth in conjugation with antacids; aluminum hydroxide and magnesium hydroxide, for symptomatic relief of pain associated with peptic ulcer disease (gastro-esophageal reflex disease).

MECHANISM OF ACTION

Oxetacaine produces reversible loss of sensation by preventing the conduction of sensory nerve impulses near the site of application. This acts by decreasing the permeability of the sodium ions across the membrane (membrane stabilizing effect).

In stomach it increases the pH and neutralizes the gastric acid, which relieves the hyperacidity problems.

Aluminum hydroxide and magnesium hydroxide react chemically to neutralize the existing quantities of acid by forming a coating over the inflamed mucosa but have no effect on production of gastric acid. Antacid also thickens the liquid to help carry the Oxetacaine and coat the food pipe.

PHARMACODYNAMICS

Oxetacaine is a topical potent anesthetic. Combining Oxetacaine with aluminum hydroxide exerts a prolonged topical anesthetic action. The antacid neutralize stomach acid and prevents further irritation of the food pipe.

DRUG INTERACTIONS

The rate and extant of absorption of certain drugs may be increased or decreased. Therefore give medicine at least 1 to 2 hours before or after a dose of this suspension. Drugs that could be affected:

  • Digoxin
  • Benzodiazepines
  • Phenothiazines
  • Tetracycline
  • Phenytoin
  • Ionized
  • Ethambutol
  • Vitamin A
  • Iron salt

An increase in the plasma level of quinidine and possible toxicity may result if alkalization of the urine occurs following antacid therapy.

CONTRAINDICATIONS

  • Antacid containing aluminum hydroxide is contradicted in patients having low level of phosphate in blood (Hypophosphatemia) due to phosphate binding property of aluminum salt.
  • Contradicted in sever renal impairment.

STORAGE

Avoid long term use of this otherwise it will leads to Hypophosphatemia.


Caution
Drinking anything immediately after taking suspension should be avoided to prevent the Oxetacaine antacid suspension being washed away before the local anesthetic has had the desire effect.
